CTUIR Vaccination Incentive Program

What is the purpose of the CTUIR Vaccination Incentive?

The CTUIR Board of Trustees approved a vaccination raffle incentive program on June 21, 2021.  The purpose of the program is to encourage community members and employees of the CTUIR to get vaccinated to help combat the COVID-19 virus.  CTUIR tribal members and employees will get their “shot” for a chance to win money and education scholarships.

What is the date of the drawing?

The drawing will take place on August 3, 2021.  Winners will be announced on the same day.

Who is eligible to win?

Prizes will be distributed as follows:

ONE TIME AWARD FOR CTUIR TRIBAL MEMBERS & EMPLOYEES

               

CTUIR Tribal Members

$100 will be given to all CTUIR Tribal members, regardless of residence, who have received at least one dose of the COVID-19 vaccine by July 30, 2021.

Employees OF CTUIR entities

Employees of CTUIR entities (Tribal Government, Yellowhawk Tribal Health Center, Wildhorse Resort and Casino (WRC includes Arrowhead, Mission Market, Hamley’s and Birch Creek Golf Course), Cayuse Holdings and Nixyaawii Community Financial Services) who have received at least one dose of the COVID-19 vaccine by July 30, 2021 AND are employed by the CTUIR entity at the time of the drawing on August 3, 2021.

EMPLOYEE RAFFLE

               

                All prizes in each category are non-transferrable.

 

$50,000 Grand Prize Drawing

All vaccinated employees of CTUIR entities who submit their name to be entered into the raffle will be entered to win a drawing for the grand prize of $50,000.  This drawing will be conducted first.  The winner of this prize will not be eligible for the remaining employee raffle drawings.  Employees of the CTUIR entities must have received at least one dose by July 30, 2021 AND be employed by the CTUIR entity at the time of the raffle drawing on August 3, 2021. 

            $2,000 Raffle Prizes

All vaccinated employees of CTUIR entities who submit their name to be entered into the raffle, will be entered into a drawing for a chance to win $2,000.  Drawings will be held separately for each CTUIR entity (Tribal Government, Yellowhawk, Wildhorse, Cayuse Holdings and NCFS).  Employees of the CTUIR entities, must have received at least one dose by July 30, 2021 AND be employed by the CTUIR entity at the time of the raffle drawing on August 3, 2021. 

The following prizes will be given away:

  1. CTUIR Tribal Government– 50 vaccinated employee names will be drawn.
  2. Yellowhawk – 15 vaccinated employee names will be drawn.
  3. Wildhorse – 100 vaccinated employee names will be drawn.
  4. Cayuse – 40 vaccinated employee names will be drawn.
  5. NCFS – One vaccinated employee name will be drawn.

CTUIR TRIBAL MEMBER RAFFLE

$2,000 Raffle Prizes

All vaccinated CTUIR tribal members 18 years of age and older on July 30, 2021 who submit their name to be entered into the raffle will be eligible for a chance to win $2,000.  CTUIR tribal members must have received at least one dose by July 30, 2021.  250 prizes of $2,000 each will be awarded in this category.

CTUIR YOUTH TRIBAL MEMBER SCHOLARSHIP RAFFLE

 

$50,000 Grand Prize Scholarship Drawing

All vaccinated CTUIR Tribal members ages 12 to 17 on July 30, 2021 who submit their name will be entered into one drawing for a grand prize scholarship of $50,000.  This drawing will be conducted first.  The winner of this prize will not be eligible for the remaining drawings in this category.  

            $10,000 Scholarship Raffle Prizes

All vaccinated CTUIR Tribal members ages 12 to 17 on July 30, 2021 who submit their name to be entered into the raffle will be entered into a drawing for a chance to win a scholarship worth $10,000.  CTUIR tribal members must have received at least one (1) dose by July 30, 2021.  25 separate $10,000 scholarship prizes will be awarded.. 

How do you enter to win?

A link/QR code, will be available for people to sign up electronically on the promotional flier.  Those who don’t have internet access can call 541-240-8500 to enter to win.

How will the winners be notified?

Winners will be contacted and have the opportunity to opt out or to provide their consent to move forward in the prize process.

Who is responsible for auditing the process?

Stauffer & Associates PLLC will oversee the drawings to ensure that there is integrity and fairness throughout the entire process.

How will you ensure the scholarship winners spend the money on college expenses?

The CTUIR Education Department will manage the scholarship prizes like other scholarships by verifying full-time enrollment and distributing funds from the account set up for the prize winner and and distributing funds from their account in equal amounts quarterly over the term that the student will be in school.  Payments will be made directly to the school.

What if the youth are not planning to attend college?

If the person chooses not to go to college then the funds shall be held until age twenty-five (25).  The funds will then be released with applicable taxes withheld and the full amount reported on a 1099-Misc.

What if I was not vaccinated at Yellowhawk?

You can still sign up to win.  All winners will need to provide proof of vaccination.  The vaccination card will not satisfy the requirement for proof of vaccination.  Winners will need to provide a signed vaccination record from the facility where they were vaccinated. .

Will you be making an accommodation for those who do not get vaccinated for health or religious reasons?

No.

How will you maintain the privacy of my information?

Each vaccinated person who signs up will be added to a database.  The system we use is HIPAA compliant.  Each name on that list is assigned a number and that number (no private data) will be drawn by Stauffer & Associates PLLC in a random, secure drawing.  The firm will maintain strict confidentiality at all times.  This would be the same process that is used for any outside vendor/contractor doing business with Yellowhawk.

How do you choose winners for each category?

Through the HIPAA compliant database system that will be used, names will be separated into the appropriate category – employee or tribal member-  and will be drawn randomly by number. 

How do you verify winners are vaccinated?

For those vaccinated at Yellowhawk or through a mass vaccination clinic conducted by Yellowhawk, Yellowhawk will provide verification..  For those who were vaccinated elsewhere, the winner must provide a written authorization for release of information for Yellowhawk to contact the place that the individual received their vaccine. 

If a winner cannot verify they have received at least one (1) dose of the COVID-19 vaccine, they will forfeit the prize.

How do you verify that winners are CTUIR Tribal members and/or CTUIR employees?

For employees, the CTUIR entity from which the employee is from will be contacted to make sure they were a current employee as of July 30, 2021 and employed at time of August 3 drawing.  If a person cannot be verified as an employee of a CTUIR entity and/or they are not currently employed at the time of the drawing, they will forfeit the prize.

For CTUIR tribal members and youth tribal members, the CTUIR enrollment office will be contacted to ensure that those winners are enrolled tribal members as of July 30, 2021.  If a person cannot be verified as a CTUIR Tribal member and/or they are no longer enrolled as a CTUIR Tribal member at the time of the drawing, they will forfeit the prize.

Will the winners’ names be made public?

Yes, upon prize acceptance, the winners’ names will be made public.  Winners have the right to decline the prize so their name will not be made public and they will forfeit the prize.

Will the prizes be taxed?

Yes, prizes are subject to appropriate federal and state taxes.  Information about tax implications will be provided by Stauffer & Associates PLLC prior to prize distribution.  Winners will be responsible for paying all applicable taxes.
