CTUIR - HEAVT Active Form

THIS FORM IS FOR ACTIVE CTUIR SCHOLARSHIP RECIPIENTS ONLY

What you need to know:

Per the CTUIR scholarship policy, all active CTUIR scholarship recipients must demonstrate good academic standing (cumulative GPA of 2.00 or higher) after each academic period and maintain a minimum Cumulative Credit Completion Rate (CCCR) of 66.67% throughout the academic year.

Failure to meet these standards will result in the recipient being placed on academic probation or suspension.

I have read and understand the eligibility standards above and agree to comply with them as a condition of applying for and receiving financial support from the Confederated Tribes of the Umatilla Indian Reservation.

What is the Continued Funding Form used for?

This form is used to verify:

Recipient eligibility status—aka passing grades or completed training hours
Recipient registration status —aka a detailed list of your in-progress classes or training sessions

Who completes the Continued Funding Form?

Active CTUIR scholarship recipients only.

An active CTUIR scholarship recipient is one who has received an official award letter from our office and has received CTUIR scholarship funding during the current academic year.
If you have not received an official award letter from our office, you are not yet classified as an active recipient in our database.
